Workplace Accommodations
- Workplace Accommodations are provided to employees to support their safe and early return to work (for those who were on a medical leave) or for employees who require additional support to maintain regular attendance at work
- Employees do not need to have been on a Medical Leave of Absence to request a Medical Workplace Accommodation
- Workplace Accommodations include temporary or permanent rehabilitation support, accommodations, assistive devices or modified work that is medically supported
- To request a Medical Workplace Accommodation employees must have the proper form (AF/APS) completed by their treating Health Care Provider and returned to the Abilities Office through fax (905) 890-0485 or email at abilities.office@peelsb.com
Please note that Medical Workplace Accommodations need to be personally medically related.
•For non-medical accommodation requests (ie. family status accommodations) please advise employees to contact their respective Employee Relations Partner for more information

Medical Leaves
•A Medical Leave of Absence is granted when an employee is in a state of incapacity due to bodily injury or illness, rendering them unable to work.
•Once an illness absence reaches or exceeds 5 consecutive days, medical documentation (Abilities Form / Attending Physician Statement / Medical Certificate) is required from the employee’s Health Care Provider to support the medical leave of absence.
•Medical Absences can be complex and all medical forms need to be reviewed by an Abilities Partner prior to a medical leave being approved.
•Medical Leaves of Absence are only considered for personal medically-related issues, therefore the Abilities Office will be unable to approve a medical leave of absence for a family-member’s illness
•Please advise the employee that they can speak to their Employee Relations Partner for other leaves that they may qualify for (ie. Family Caregiver Leave)
